Like many people/ organisations that provide teaching services or leadership, I am interested in knowing where I “hit the mark” versus “miss the mark” when it comes to:
a) The relevancy of the learning material/ resources that I provide to people who I am teaching/ leading, and
b) The delivery of the information that I verbalise to those who I am teaching/ leading.
So I’ve put together this simple Feedback Form:
…to provide the opportunity for students who I teach at the Eastern Institute of Technology to tell me their honest thoughts when it comes to a) and b) above. There’s certainly no obligation to do so.
If you’d like to let me know how you view the value of the teaching lessons that you receive from me, please follow these steps…

Step Two – save the Feedback Form to your local drive/ desktop (to then be able to send it back to me as an email attachment).
Step Three – enter whatever feedback you’d like to communicate to me in this form, and save what you have entered.
